REQUIRED CAMP FORMS
Prior to the start of camp, we require the following forms ( Waiver, Medical Form) to be completed.
No Participation will be permitted without both forms completed prior to any physical activity at camp.
Camp Waiver (Mandatory)
The camp waiver can be completed online or by downloading, printing and bringing to camp check-in the first day.
